Site - On or about October 22, 2020, employees engaged in framing activities were exposed to falls of over 8 feet while working from the second floor level, where fall protection was not provided or utilize to prevent the hazard.  Instance b. Site - On or about October 22, 2020, employees engaged in framing activities were exposed to fall hazards, where floor openings were not guarded or covered to prevent the hazards.

Site - On or about October 22, 2020, employees were exposed to fall hazards while attempting to navigate the eight-foot (Werner) stepladder to access the second floor level, where the ladder did not extend 3 feet above the upper landing surface.
