29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  9203 Griffith Run:  On or about October 06, 2020, and at times prior thereto, employees were exposed to a struck-by hazard while operating a pneumatic nail gun without wearing eye protection.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   Commercial Apartment Complex Roof at 9918 Hazy Bend: On or about October 30, 2020, employees were exposed to a fall hazard while installing decking panels at a height greater than 6 feet from the surface below, without a fall protection system.
