The employer did not provide and require employees to wear safety glasses or face shields.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Dingo Drive: On November 5, 2020 and times prior thereto the employer exposed employees to fall hazards by allowing employees to use a portable ladder to access a upper landing surface without extending the portable ladder side rails at least 3 feet (.9m) above the upper landing surface being accessed. The employees were exposed to fall hazards of 12 feet 4 inches to the ground below when using the ladder.
