29 CFR 1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Concrete crushing area - On or about October 19, 2020, adequate barriers were not in place to prevent employees from being struck by dislodged concrete debris. Abatement note: Among additional feasible means of protecting employees from falling debris would be to conduct general housekeeping of accumulated metal and rebar and relocating the crushing and screening line farther from the concrete piles.
