29 CFR 1926.451(c)(2):Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation. a) J Paul Drywall, LLC, at a jobsite located at 856 Hebron Road, Heath, Ohio 43056: On and/or preceding November 5, 2020, the employer did not ensure the upright members of a mobile scaffold were placed on mud sills or other adequate firm foundation. An employee was working from the second and third levels of a mobile scaffold where the vertical members had casters installed, which were placed directly on the ground and on a piece of plywood, thereby being exposed to fall hazards of up to 18 feet.

29 CFR 1926.451(g)(1):Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. a) J Paul Drywall, LLC, at a jobsite located at 856 Hebron Road, Heath, Ohio 43056: On and/or preceding November 5, 2020, the employer did not ensure employees were protected from falls while working from a scaffold at heights greater than 10 feet. 29 CFR 1926.452(w)(1):Scaffolds shall be braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. An employee was working from the second and third levels of a mobile scaffold where the vertical members were not secured and the scaffold was not plumb and level, thereby being exposed to fall hazards of up to 18 feet.
