This was most recently documented on or about November 6, 2020 at 9625 South 71st Street, Lincoln, Nebraska when employees were observed working on a residential structure with a roof pitch greater than 4:12. The employees where installing shingles while working at a height of approximately 10 feet 10 inches measured from the ground to eave without fall protection.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: The employer is failing to train all employees that were potentially exposed to fall hazards.
