On or about November 6, 2020, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing employees to hazards associated with eye injuries.  Lakeshore Roofing Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1147252, citation number 1, item number 1, and was affirmed as a final order on June 15, 2016, with respect to a workplace located at Lot 49 Jennifer St. in Frankfort, IL.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about November 6, 2020, at the above addressed jobsite, an employee performing residential roofing operations, eight (8) feet seven (7) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about November 6, 2020, at the above addressed jobsite, an employee performing residential roofing operations, thirteen (13) feet one (1) inch above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about November 6, 2020, at the above addressed jobsite, employees performing residential roofing operations, fourteen (14) feet five (5) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  Lakeshore Roofing Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1147252, citation number 1, item number 2, and was affirmed as a final order on June 15, 2016, with respect to a workplace located at Lot 49 Jennifer St. in Frankfort, IL.

On or about November 6, 2020, at the above addressed jobsite, the portable ladder used to access the garage roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.  b.  On or about November 6, 2020, at the above addressed jobsite, the portable ladder used to access the house roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
