29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) Avalanche Quality Construction LLC at 4240 Pebble Ridge Dr. Colorado Springs, CO 80906: On or before 11/6/2020, the creating and exposing employer did not protect employees from injury in that employees accessed the first story roof over the laundry room using a portable extension ladder which did not extend at least three feet beyond the eave of the roof.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Avalanche Quality Construction LLC at 4240 Pebble Ridge Dr. Colorado Springs, CO 80906: On or before 11/6/2020, the creating and exposing employer did not protect employees from injury in that employees working on the first and second story of a two story townhome building were conducting tear off operations without wearing fall protection. This condition exposed employees to fall hazards of 9 and 17 feet. Avalanche Quality Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1481777, citation number 1, item number 1, and was affirmed as a final order on 08-31-2020, with respect to a workplace located at 16950 Carlson Dr. Parker, CO 80134.
