29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. Construction Project, 18 Main Dock, Rd. 3 - A black, foldable chair was used in lieu of stairs to gain access to the main point of entry to the interior of the project. Employees are exposed to slip, trip, and fall hazards.

29 CFR 1926.451(g)(4):Guardrail systems installed to meet the requirements of this section shall comply with the following provisions (guardrail systems built in accordance with Appendix A to this subpart will be deemed to meet the requirements of paragraphs (g)(4)(vii), (viii), and (ix) of this section): Construction Project, 18 Main Dock, Rd. 3 - The following inadequacies were observed on A-1, Modular Series scaffolding erected at the worksite. Employees were exposed to fall hazards up to 12'. 1. Main deck level, facing front from access point - An entire section (approximately 8 feet) of standard guardrails, including bracing, was removed to allow onboarding of lumber; 12' fall hazard to ground level.
