29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6-feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of '1926.502(k) developed and implemented.  On or about November 06, 2020, and at times prior thereto, at a workplace located at 1215 Remington Ranch Road, Mansfield, Texas; an employee replacing window beading was walking and working from the roof and was not protected from falling more than 9 feet to the lower level by use of guardrail or personal fall arrest system.

29 CFR 1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails was not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about November 06, 2020, and at times prior thereto, at a workplace located at 1215 Remington Ranch Road, Mansfield, Texas; an employee replacing window beading used a ladder that was not extended at least 3 feet above the upper landing to access the working area.
