29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about 11/9/20, the employer failed to create, implement, and maintain a lockout/tagout program. Employees were conducting maintenance, repair, and cleaning tasks on machinery including, but not limited to the following: a) Line #1 on the third floor, types of energy include pneumatic, electric, and thermal, b) Line #2 on the third floor, types of energy include pneumatic, electric, and thermal, and c) The Automatic Capsule Filling Machine, Model # TES-1200, serial # 121402. Types of energy include pneumatic, and electric.

The Omega Design Corporation Model 3D-ERP2-10, serial # 98-J23523, had a bypassed safety interlock exposing employees to pinch points, caught in, and struck by hazards.
