29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section. (a) Jobsite located on 230 E. Berkley Ave. Clifton Heights, PA - 19018: On or about November 6, 2020, the employer did not ensure that fall protection was being used by employees performing roofing activities approximately 11 feet above ground level, thereby exposing them to fall hazards. Middle Creek Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.501(b)(13), which was contained in OSHA inspection number 1489397, citation number 1, item number 1 and was affirmed as a final order on August 25, 2020, with respect to a workplace located at 935 Brookside Rd. Allentown, PA - 18106. Middle Creek Roofing LLC was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA inspection number 1211582, citation number 1, Item number 1 and was affirmed as a final order on July 19, 2017, with respect to a workplace located at 15 Ladybug Lane, Meyerstown, PA.

Berkley Ave. Clifton Heights, PA - 19018: Employees performing roofing activities located approx. 11ft above ground level were connected to the same Guardian Temper roof anchor. Per manufacturer's guidance, the anchor is only designed for a maximum of 1 personal fall arrest system (PFAS). Condition was observed on or about November 9, 2020.

Berkley Ave. Clifton Heights, PA - 19018: The employer failed to provide fall protection training to all employees who may be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed. Multiple employees were attached to one anchor point. Thus exposing the employees to fall hazards. Condition noted on our about November 9, 2020.
