29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  a)  At the jobsite, on the warehouse roof, on or about 11/9/20:  Employees working on the roof were not protected from falling through skylights, comprised of translucent fiberglass panels, by covers or any type of fall protection system.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-sloped roofs with unprotected sides and edges 6 feet (1.8m) or  more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line systems and safety net systems, or warning line systems and personal fall arrest systems, or warning line systems and safety monitoring systems:  a)  At the jobsite, on the warehouse roof, on or about 11/9/20:  Employees were working on a low slope roof with a ground to eave height of 19 feet without fall protection of any kind.
