29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): 195 Ashburnham St., Fitchburg, MA: On or about November 10, 2020 employees engaged in roofing activities and exposed to falls greater than 23 feet were not protected from falling by guard rail systems, safety net systems or a utilized personal fall arrest system (PFAS). Ascend Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926501(b)(1), which was contained in OSHA inspection number 1258074, citation number 1, item number 9b and was affirmed as a final order on January 8, 2018 , with respect to a workplace located at 4 Horman Road N Billerica, MA 01862.
