29 CFR 1926.405(d):Panelboards mounted in cabinets, cutout boxes or enclosures designed for the purpose were not dead front: a) Inside the storage building along the front wall, on or about November 10, 2020 and at times prior to, an employee flipped a breaker on the breaker panel cabinet that was not dead front, in that no blank was installed where a breaker was missing, resulting in an electrical shock hazard.

29 CFR 1926.405(e)(1):Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures: a) Inside the storage building, on or about November 10, 2020 and at times prior to, where employees were installing rafters, the energized 120-volt electrical system including fittings and boxes designed for dry conditions was exposed to damp and wet conditions as a result of the roof being removed.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Along the front of the storage building, on or about November 10, 2020, the employer did not ensure that a 20' extension ladder was set up safely for an employee who was accessing the rafters, in that the ladder only extended two feet above the eave.
