29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: At a worksite located at 1201 NW Indian Lane Blue Springs, MO 64015: Employees were exposed to fall hazards. This was demonstrated on or about October 28, 2020 when a worker fell approximately 24 feet to the ground below. The fall resulted in several serious injuries that included, but were not limited to cranial fracture, brain bleed, and several fractures to the upper extremities. The employer failed to ensure that workers were properly trained to understand and avoid unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: At a worksite located at 1201 NW Indian Lane Blue Springs, MO 64015: Employees were exposed to fall hazards. This was demonstrated on or about October 28, 2020 when a worker fell approximately 24 feet to the ground below. The worker suffered serious injuries as a result of their fall and they had to be hospitalized. The employer failed to ensure that workers were not exposed to unprotected sides or edges by providing protective systems to prevent the workers from falling.

At a worksite located at 1201 NW Indian Lane Blue Springs, MO 64015: On or about October 28, 2020 a worker fell approximately 24 feet to the ground below. The fall resulted in several serious injuries resulting in the worker having to be hospitalized as a result of these injuries. The employer failed to report that hospitalization to OSHA with-in 24-hours.
