Employees wear a supplied air RPB Safety model NOVA 2000 type CE respirator helmet during abrasive blasting. Employees wear tight-fitting 3M N95 and 3M 6900 full-face respirators with 3M 60926 P100 filters during welding and painting. The employer had not established and implemented a written respiratory protection program. This condition exposed employees to a respiratory hazard.

An employee had medium thickness facial hair and wore a tight-fitting N95 respirator while wiping aluminum with cleaning solvents. This condition exposed the employee to a respiratory hazard.

Employees were required to wear 3M model 6900 full-face mask air purifying respirators with 3M model 60926 P100 filters when welding or painting. In addition, employees were required to wear a RPB Safety model NOVA 2000 type CE respirator when abrasive blasting. The employees did not receive comprehensive respirator training. This condition exposed employees to respirable crystalline silica, hexamethylene diisocyanate (HMDI), and hexavalent chromium hazards.

Employees performed painting, cleaning, welding, and abrasive blasting operations on aircraft parts. This condition exposed employees to chemical hazards. A spray bottle containing a clear liquid had handwritten on its side "IPA" and nothing else. The secondary container was not provided with a label nor was marked with the identification of hazard information of the content. This condition exposed employees to a chemical hazard. b) Paravion Technology, Inc., at 2001 Airway Avenue, Fort Collins, CO 80524: On and before November 10, 2020, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the correct name of the chemical and appropriate hazard information. A container previously containing Startex paint thinner, but re-used to contain a different chemical, was identified with the name "look" written on the container. The re-used container was not provided with a label nor was marked with the identification or hazard information of the content. This condition exposed employees to a chemical hazard.
