29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) Collins Pine Company, Kane Hardwood Division; conveyor to stacker; sorter area - On or about 11/12/20, at times prior thereto, the conveyor was not locked out prior to employees being near the conveyors, unguarded chains and sprockets when performing servicing and maintenance. The area was being entered for employees to perform work including, but not limited to, moving lumber from chain 1 to chain 2 of the conveyor. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR 1910.265(c)(18)(i): Construction, operation, and maintenance of conveyors were not in accordance with American National Standard B20: a.) Collin Company, Kane Hardwood Division; sorter area -- Chain/Sprocket Associated with the Conveyor - - On or about 11/5/20, and at times prior thereto, chains and sprockets were not guarded to prevent employees from getting caught in them. An accident occurred in which an employee's foot was pulled into the sprocket area causing the employee to sustain a 2 toe amputation. One or more methods of effective machine guarding were not provided to protect employees in the conveyor area from hazards such as those created by ingoing nip points and rotating parts from chains and sprockets. In addition, the perimeter guarding was not so designed and constructed as to prevent the employees from being in the areas by the chain/sprocket in that employees could by-pass the perimeter guarding by utilizing two different stairways. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1910.219(c)(2)(i):All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: a.) Collin Company, Kane Hardwood Division; sorter area - On or about 11/12/20, and at times prior thereto, two(2) shafts were unguarded. The shafts ran from the motors to the chain and sprockets on the line and were at ground level. In addition, the perimeter guarding was not so designed and constructed as to prevent the employees from being in the areas by the power transmission apparatus in that employees could by-pass the perimeter guarding by utilizing two different stairways. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Collin Company, Kane Hardwood Division; sorter area -- Chain/Sprocket Assembly by Motor - - On or about 11/5/20, and at times prior thereto, chains and sprockets were not guarded to prevent employees from getting caught in them. The chain/sprocket assembly were located at ground level. In addition, the perimeter guarding was not so designed and constructed as to prevent the employees from being in the areas by the power transmission apparatus in that employees could by-pass the perimeter guarding by utilizing two different stairways.
