29 CFR  1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   a) Job site, 200 Water Street, Rochester, PA: On or about November 10, 2020, during demolition activities, the employer did not ensure that employees used appropriate fall protection systems/devices near the edge of a building foundation where an approximate 20 foot  or so fall hazard existed.

29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:   a) Job site, 200 Water Street, Rochester, PA: On or about November 10, 2020, during demolition activities, the employer did not ensure that an employee used appropriate fall protection systems/devices where working near a hole in a large section of floor grating, exposing the employee to a fall hazard of up to approximately 20 feet or so.

29 CFR  1926.850(i):  All floor openings, not used as material drops, were not covered over with material substantial enough to support the weight of any load which may be imposed and such material was not properly secured to prevent its accidental movement:    Instances including, but not limited to, the following:  a) Jobsite, 200 Water Street, Rochester, PA: On or about November 10, 2020, during demolition activities, the employer did not ensure that the plywood placed over an opening in a grate determined to be approximately 18 inches wide by 4 feet long was secured in place to prevent movement, exposing an employee to a fall hazard of approximately 20 feet or so.  b) Job site, 200 Water Street, Rochester, PA: On or about November 9, 2020, during demolition activities, the employer did not ensure that covers were provided over floor openings not in use for material drops, such as one determined to be approximately 10 inches wide by 18 inches long, exposing employees to hazards such as stepping into it.
