29 CFR  1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  A.)  Rabe Environmental Systems, Inc., 2881 Mechanic Street, Lake City, PA  16423, on or about November 9, 2020 and times prior thereto:  Employees were performing tasks associated with the installation of HVAC units.  Employees were not utilizing an acceptable means of method of fall protection when exposed to a fall hazard of 18.1-feet to the ground below.  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR 1926.503(a)(2):  The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  A.)  Rabe Environmental Systems, Inc., 2881 Mechanic Street, Lake City, PA  16423, on or about November 9, 2020 and times prior thereto:  Employees were performing tasks associated with the installation of HVAC units and were exposed to a fall hazard.  The fall protection training program (section 26) within the Rabe Environmental Systems Safety Standards (6G0.1.1) document was deficient in that the program did not address topics including, but not limited to:  1.)  The nature of fall hazards in the work area; 2.)  The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems including, but not limited to, guardrail systems and personal fall arrest systems; 3.)  The use and operation of fall protection systems including, but not limited to, guardrail systems and personal fall arrest systems. 4.)  Skylights 5.)  Fixed ladders  ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation.
