Location: 710 Metropolitan Ave, Brooklyn, NY.  On or about 11/12/2020,  a)  Employees anchored their personal fall arrest equipment to the scaffold components during dismantling. No determination if the anchorage was capable of supporting at least 5,000 was done.

The employer did not provide fall protection for employees erecting or dismantling supported scaffolds.   Location: 710 Metropolitan Ave, Brooklyn, NY.  On or about 11/13/2020,  a)	The employer did not determine the feasibility of using fall protection during the erection of a supported scaffold and employees did not use any fall protection when the use of personal fall arrest systems were feasible.  Employees were wearing harnessess but were not tied off therefore exposed to a fall of approximately 50 feet.

29 CFR  1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  The training provided did not include the nature of scaffold hazards.    Location: 710 Metropolitan Ave, Brooklyn, NY.  On or about 11/12/2020,  a) The Employer did not provide training to employees on fall protection when erecting and dismantling a scaffold.   Training should include but not be limited  to inspect their personal fall arrest systems (PFAS) prior to use, and to select an anchorage point which is capable of supporting at least 5,000 lbs. per person.
