29 CFR 1926.451(c)(2)(v):The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a. At a temporary residential construction worksite, located at 6595 San Juan Avenue in Jacksonville, Florida: On or about November 13, 2020, the employer exposed employees to 15-foot fall hazards, in that a wood pallet that was used by employees as a scaffold platform and the platform was not attached to the fork of the forklift to prevent the platform from shifting, as the employees performed roofing work.

29 CFR 1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. At a temporary residential construction worksite, located at 6595 San Juan Avenue in Jacksonville, Florida: On or about November 13, 2020, the employer exposed employees to 15-foot fall hazards, in that fall protection was not utilized by employees who stood on wood pallet that was used as a scaffold platform and elevated by a forklift, while preforming roofing work.
