29 CFR 1926.451(c)(2):Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation. a) On or about November 12, 2020, at the previously addressed job site, a supported scaffold was erected with its posts bearing on a wood subfloor without base plates/mud sills and later loaded with mason blocks. The posts of the loaded side broke through the wood floor and the scaffold overturned while an employee was working from the scaffold. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.451(c)(2)(i):Footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement. a) On or about November 12, 2020, at the previously addressed job site, a supported scaffold was erected without base plates/mud sills on a wood subfloor and later loaded with mason blocks. There were no footings capable of supporting the loaded scaffold when the scaffold posts broke through the wood subfloor and tipped over with an employee working from the scaffold. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
