29 CFR  1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  The employer is failing to protect employees from fall hazards while on a scaffold. This was most recently documented on or about November 5, 2020 at 21423 B St., Elkhorn, Nebraska when an employee was observed on a scaffold that was not fully planked thus decreasing  the working area for the employees.  The employee was working at a height of over 10 feet without fall protection.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.  The employer is failing to protect employees from fall hazards when installing stone veneer on the exterior of a residential structure. This was most recently documented on or about November 5, 2020 at 21423 B Street, Elkhorn, Nebraska when an employee was observed working on a residential structure. The employee was performing stone work application while working at a height of over 10 feet without fall protection.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  The employer is failing to protect employees from fall hazards when installing stone veneer on a residential structure. This was most recently documented on or about November 5, 2020 at 21423 B St., Elkhorn Nebraska when employees were applying stone veneer on a residential structure without fall protection. The employees were working at a height of over 13 feet.
