29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (b) On or about October 15, 2020, employees were exposed to struck-by and crushing hazards created by working inside a trench without an adequate protective system in place. Employees worked in a trench box that was approximately 8 feet high which was placed inside an excavation that was 13 1/2 feet deep. Leo's Sewer & Drain, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1454029, citation number 1, item number 1 and was affirmed as a final order on February 18, 2020, with respect to a workplace located at an excavation located in front of 4448 White Oak Ct., Colorado Springs, CO. Leo's Sewer & Drain, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) , which was contained in OSHA inspection number 1340240, citation number 1, item number 1b and was affirmed as a final order on October 1, 2018, with respect to a workplace located at an excavation located at 1535 S. 8th Street, Colorado Springs, CO.
