29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about November 20, 2020, employees were exposed to machine hazards associated with moving parts, rotating parts and ingoing nip points during cleaning activities at SP1. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the cleaning work [per 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about November 17, 2020, located at 180 count, there was a conveyor with approximately 2-1/2 inch opening at its end roller that was not guarded to protect employees from the ingoing nip point between the belt and roller, thereby exposing employees to a caught-in hazard. b. On or about November 20, 2020, located at SP1, there was a conveyor with approximately 1-1/2 inch opening at its end roller that was not guarded to protect employees from the ingoing nip point between the belt and roller, thereby exposing employees to a caught-in hazard. c. On or about November 20,2020, employees were exposed to ingoing nip points associated with the motor on the product settler underneath SP1.

The guard shall be such that it does not offer an accident hazard in itself. a. On or about November 17, 2020, located at 180 Count, there was a Belkin, model number F5C048-2, relocatable power tap (RPT) that was not directly connected to a permanently installed branch-circuit receptacle outlet. The RPT was used in part to supply electrical power to a Belcor tape machine, model number 150S and a Domino inkjet printer, model number Ax350i.
