29 CFR  1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:   a) Throughout the establishment:  On or about November 12, 2020, and at times prior, the employer exposed employees to engulfment hazards in that a written confined space program had not been established or implemented for employees who may have to enter tanks and/or vats.

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Warm Storage Building:  On or about November 13, 2020, and at times prior, the employer exposed employees to chemical burn hazards, in that emergency means for flushing the eyes and the body was not provided for employees working with chemicals, such as, but not limited to, caustic soda, phenol and hydrochloric acid.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   a) Throughout the establishment:  On or about November 18, 2020, and at times prior, the employer exposed employees to chemical hazards, in that  hazard communication program training was not provided to employees working with chemicals, such as but not limited to, caustic soda, phenol, ethanol, isopropyl alcohol, Pullulan, and Dextran.  HPPE LLC was previously cited for a violation of an equivalent occupational safety and health standard 1910.1200(h)(3), which was contained in OSHA inspection number 1343986, citation number 1, item number 2 and was affirmed as a final order on February 05, 2019 with respect to a workplace located at 6906 Dixie Street, Columbus, Georgia.
