29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: On or about 11/6/2020 at 2190 Northlake Parkway Tucker, GA where a worker was observed exposed to fall hazards while using a folded six foot step ladder to elevate his position 18-inches above the balcony level. The employer failed to ensure the proper size and type ladder was available and used during light fixture installation activities on the second floor balcony.

29 CFR 1926.404(f)(6):The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about 11/6/2020 at the temporary power pole at the rear of the site closest to highway 285, workers of all trades were exposed to electrical shock and burn hazards where the duplex, GFCI outlet was not properly grounded. The ground wire was not connected for the past four months.
