29 CFR 1926.405(a)(2)(ii)(I):Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: a) Office Trailer, 3200 Capri Drive, Philadelphia, PA - The employer did not ensure that all temporary energized electrical feeder cables running through the window of the job trailer was protected from pinch points and sharp edges. As a result, employees were exposed to fire, smoke, and electrical hazards. Observed on or about 23 JULY 2020.

29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Jobsite - The employer did not ensure that the lanyard used for fall protection was capable of stopping a falling employee. The employer allowed employees to use a forty-four (44) feet lanyard with a five (5) feet shock pack attached when working at heights of approximately forty-four (44) feet when framing the fourth floor of a new residential building. Observed on or about 23 JULY 2020. Acosta Services, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(13), which was contained in OSHA inspection number 1449211, citation number 1, item number 2 and was affirmed as a final order on 05 FEB 2020, with respect to a workplace located at 3200 Capri Drive Philadelphia, PA 19145.
