29 CFR  1910.305(b)(1)(ii):  Unused openings in boxes, cabinets, or fittings were not effectively closed:  At this establishment, electric circuit panels and electrical outlets had unused openings in the following instances:  a.  main shop building, two electric circuit panels had openings on the bottom of the panels; b.  main shop building, an electrical panel had a missing circuit breaker #20; c.  drill press shop, an electrical outlet had an opening on the bottom of the outlet box; d.  code shop, an electrical circuit panel missing blanks/breakers #5, #6, and #9, and a second electrical circuit panel missing blanks/breakers #15 and #16; and, e.  code shop, a Square D general duty safety switch, 100A, 240V, for a Flextec 650X welder, had an opening on the bottom of the box.
