The employer did not ensure that manlift(s) were intended for conveyance of persons only: In the grain elevator at the man lift, employees engaged in grain handling activities were exposed to struck-by hazards in that the man lift steps were used to transport tools.

The employer failed to guard against a person getting between the mounting platform and an ascending step, the area between the belt and the platform shall be protected by a guardrail: In the grain elevator at the man lift, employees engaged in grain handling activities were exposed to struck-by hazards, in that there was no guard or rail to prevent a person from getting between the mounting platform and the ascending step.
