29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about November 16, 2020, the employer did not develop machine specific procedures for the Van Dorn 400 Plastic Extruder, therby exposing employees to hazards associated with the unexpected re-energization of equipment. Energy sources associated with equipment include, but are not limited to electrical, pneumatic, and hydraulic.

Maintenance employees were not trained on the recognition of hazardous energy sources and the methods and means necessary to control them. These employees were exposed to machine hazards that could result in laceration and amputation injuries.
