29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  BUILDING #853: On or about November 17, 2020 an employee engaged in roofing activities on the low-slope roofs of single story buildings was not tied off and there was no anchor, exposing the employees to falls of 12 feet to the ground below.

29 CFR  1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  BUILDING #858: On or about November 17, 2020 Employees engaged in roofing activities on the low-slope roofs of single story buildings were using 3 foot long shock absorbing lanyards connected to 20 foot long life lines, exposing the employees to falls of 12 feet to the ground below.
