29 CFR  1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.   A)   Lamps Inc. dba ERG Environmental Services, worksite located 525 Industrial Drive, Wauseon, Ohio, 43567: On or about November 18, 2020: The employer did not ensure an employee operating a fork truck was adequately trained.  An evaluation of competency for the employee operating the forklift was not conducted and documented by the employer. The observed operating employee and others working around the forklift were potentially exposed to struck-by injuries due to operation by an unevaluated operator.

29 CFR 1910.184(e)(1): Alloy steel chain sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:  A)	Lamps Inc. dba ERG Environmental Services, worksite located 525 Industrial Drive, Wauseon, Ohio, 43567:  On or about November 18, 2020,  employees were utilizing a chain sling without permanent identification to hoist loading dock ramps. Without the benefit of this information identifying lifting capabilities and limitations, employees using the chain in this condition were exposed to struck-by hazards.

29 CFR  1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:   A)   Lamps Inc. dba ERG Environmental Services, worksite located 525 Industrial Drive, Wauseon, Ohio, 43567: On or about November 18, 2020:  The employer did not ensure the pallet racking system on the north side of the building was stable and secure. The pallet racking system's base plates were not positioned completely on the flat concrete surface and securely fastened and/or anchored.
