29 CFR 1904.31(b)(2): Employees from a temporary help service supervised on a day-to-day basis sustained an injury or illness which was not recorded on the OSHA 300 log(s): Calavo Growers Inc. located at 100 Dartmouth Drive, Ste 500 Swedesboro, NJ: a) On or about 12/07/2020 an employee placed by AMP Staffing was injured while sorting avocados on a moving machine. The employee received deep lacerations resulting in 6 stitches and lost work time. The employer did not record the work-related injury on the OSHA 300 log or its equivalent for the 2020 calendar year. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
