None of the equivalent forms included a reference to the OSHA 300 Log Case Number.  Some of the forms did not include the part of body and actual injury.  The equivalent forms provided for the injuries that took place on 11/16/2020, 11/17/2020, 11/17/2020, and 12/2/2020 did not include employee address, employee birthday, or information about medical treatment facility.  No equivalent form was provided for the injury that took place on 11/24/2020.
