29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 18 November 2020, at the above addressed jobsite, two employees were in the process of removing underlayment from the roof of an existing home under renovation, without the use of fall protection, exposing the employee to a fall hazard of approximately 8 feet.

29 CFR 1926.960(c)(1)(i): The employer did not establish minimum approach distances (MAD) no less than the distances computed by Table V-2 for ac systems: On or about November 18, 2020, at the above residence address, two employees accessed a ladder that was approximately 8 inches away from a weather head and worked near it installing plywood while utilizing hammers and electric table saw, exposing the employees to a shock hazard.
