(a) 129 Bickley Rd Glenside, PA., Roof: On or about November 18, 2020, employees working on the ground were exposed to head injuries from material being thrown from the roof. Employees on the ground were not wearing head protection in the form of hardhats.

29 CFR  1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  (a) 129 Bickley Rd Glenside, PA.: On or about November 18, 2020, employees used a 24-foot Louisville extension ladder to access the roof of a two-level residential structure, however, the ladder was damaged (with two bent shoes/braces).
