(a) 129 Bickley Rd Glenside, PA., Roof: On or about November 18, 2020, employees working on the ground were exposed to head injuries from material being thrown from the roof. Employees on the ground were not wearing head protection in the form of hardhats.

(a) 129 Bickley Rd Glenside, PA.: On or about November 18, 2020, employees used a 24-foot Louisville extension ladder to access the roof of a two-level residential structure, however, the ladder was damaged (with two bent shoes/braces).
