29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) McKinely Robotic Cell: On or about November 13, 2020, sufficient guarding was not provided to protect the operator, and other employees working in the area from access to the energized components of the Robotic cell. This exposed employees to point of operation, ingoing nip points, rotating parts and amputation injury.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: (a) McKinely robotic Cell: On or about November 13, 202o sprocket wheels and chains located approximately eight (8) to 12 inches above the floor were not enclosed in a manner to prevent employee access. This exposed employees who entered the energized cell through an unguarded section of the cell enclosure to a caught-in and crushing hazard, and amputation injury from the exposed components.
