29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a)	VFA -106, Oceana Naval Air Station: On or about 11/18/2020, the employer failed to conduct a hazard assessment to determine the proper personal protective equipment or labor saving device for the removal of heavy floor grating, as a result an employee's right middle finger tip was amputation.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
