On or about November 18, 2020, at a worksite located at 3771 Harris rd. Broadview Heights, OH 44147, the employer did not ensure employees had means of access and egress while working inside a trench that had near vertical side walls and a depth of approximately 8 feet.

29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about November 18, 2020, during the replacement of a sanitary line at a worksite located 3771 Harris rd. Broadview Heights, OH 44147, the employer did not ensure two employees working inside a trench, with an approximate depth of 8 feet , were protected from cave-ins by an adequately designed protection system. The soil was classified as Type �B� soil.
