29 CFR  1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  On or about November 17, 2020, the employer failed to ensure that employees tasked with performing service and maintenance of various equipment and machinery, to include but not limited to, the Ormond Fillers and Feeders, were trained as authorized to perform lock out.

29 CFR  1910.147(c)(4)(ii)(D):The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control:    On or about November 17, 2020, the employer failed to ensure that their lock out tag out procedures for machines and equipment, to include but not limited to, the Ormont Filler and Feeders, included checking to ensure the energy of said machines was completely locked out prior to allowing employees to clear jams and perform other tasks.  Such as identifying within the procedures, the stop time of the impeller fan.  NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1424464, citation 1, item 1a, which was affirmed as final order on October 29, 2019, with respect to the workplace located at 9759 Church Road West, Walls, MS 38680.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   On or about November 17, 2020, the employer failed to conduct an audit at least annually on their written lock out tag out procedure to ensure its efficacy.  The last documented review was dated in 2015.    NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1424464, citation 1, item 1c, which was affirmed as final order on October 29, 2019, with respect to the workplace located at 9759 Church Road West, Walls, MS 38680.

There are only two locks being used and no tags so it is unclear as to who is performing the service work or clearing of a jam on equipment and machinery, to include but not limited to, the Ormond Fillers and Feeders.      NOTE:  The employer was previously cited for a violation of this occupational safety and health standard or it's equivalent standard which was contained in OSHA Inspection #1424464, citation 1, item 1b, which was affirmed as final order on October 29, 2019, with respect to the workplace located at 9759 Church Road West, Walls, MS 38680.
