On or about November 20, 2020, on the roof of a residential home, employees were exposed to struck by hazards while operating a nail gun without eye protection.

On or about November 18, 2020 and times thereafter, employees were exposed to a fall hazard greater than 6 feet when performing roofing work without a fall protection system.
