On the west side of the work site, the employer did not ensure that employees inspect electrical tools and equipment and used an extension cord while performing painting activities that was missing the ground pin, exposing employees to electrical hazards.

On the west side of the work site, an employee engaged in painting activities from the roof without fall protection was expose to a fall hazards of approximately 15-feet.
