29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On the west side of the work site, employees engaged in framing activities without fall protection were exposed to fall hazard of approximately 10 feet to the ground below. Ramiro Zapata Castillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1322867, citation number 01, item number 001 and was affirmed as a final order on August 15, 2018, with respect to a workplace located at 7730 Holloway Dr., Irving, Texas 75063.

29 CFR 1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On the west side of the work site, employees engaged in framing activities utilized an extension cord to energize a Makita skill saw with the outer sheathing pulled away exposing internal wires. Employees were exposed to shock hazards.
