29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts on grinding machinery: a) Fabrication area in the shop: On or about November 24, 2020, machinists used a Baldor grinder CAT#623E, serial number W190, that had an unguarded spindle end, nut and flange that rotated when the machine was in use.
