29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. Saw Mill Building - On November 17, 2020 an employee suffered an amputation of three fingers when the employee was performing repairs on the bin sorter without having the equipment locked out at the energy isolating device. Another employee activated the equipment while the victim was performing repairs.
