29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs and protected by traffic control devices at points of hazards: (a) Parking Lot at Southwest Corner of Building - An employee worked installing and integrating a rooftop telecommunications antennae from a JLG 1350SJP Ultra Boom aerial lift stationed in the parking lot. No traffic signs nor traffic control devices got conspicuously posted/installed to alert and channel approaching traffic to protect the aerial lift from getting struck by the vehicles traveling in the parking lot, on or about November 19, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.453(a)(2): Field modification of the aerial lift was not certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969: (a) Parking Lot at Southwest Corner of Building - Employees performed an unapproved field modification to the rented JLG 1350SJP Ultra Boom aerial lift, serial number 0300183075, by attaching a nylon sling to the underside of the lift's basket/platform in order to use the aerial lift to lower old telecommunication antennas removed from the building and to raise new telecommunication antennas for installation on the building, exposing employees to hazards such as tipping and struck by, on or about November 19, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Northwest Corner of Roof of Two-Story Extension on Front of Building - An employee stood on the two-story extension's roof to install and integrate a rooftop telecommunications antennae attached to the exterior northwest corner wall of the two-story extension. The roof had a parapet wall that came up only to about mid-thigh level on the employee, and at times, the employee laid on top of the parapet wall to work. The employee did not have protection from falling to a lower level by a guardrail system, safety net system, or personal fall protection system, on or about November 19, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee: (a) Southwest Side of Building - Two employees rode in the basket of the JLG 1350SJP Ultra Boom aerial lift to come from the roof level down to ground level. Both employees attached the lanyards of their personal fall arrest systems to the same anchorage point within the aerial lift's basket, contrary to the JLG Operation and Safety Manual, which says to attach one lanyard to each lanyard anchorage point, exposing the employees to the hazard of falling to a lower level, on or about November 19, 2020. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
