On or about November 16, 2020: The forklift driver was observed not using the seatbelt affixed to the forklift.

29 CFR 1910.22(d)(1):Walking-working surfaces were not inspected, regularly, and maintained in a safe condition. a) On or about November 16, 2020 the rig platform near the ground was observed improperly supported creating an uneven surface leading to a tripping hazard. b) On or about November 16, 2020: The stairs leading to the mud pit where observed being supported by lose boards. The boards can be easily removed or displaced potentially causing the stair case to collapse.
