A) 1365 North Ave Elizabeth, NJ 07208  The employer did not ensure all gas utilities were located in a safe and acceptable way. A 2 inch gas line was struck by the bucket of an excavator  and causing a gas leak.   Condition noted on or about 11/24/2020
